From Data to Decision: Embedded Decision Support

At the core of VaxyReach is an embedded Clinical Decision Support Tool (CDST) that supports frontline decision-making. It combines national immunisation guidelines, programmatic rules, and simple, transparent prioritisation logic to provide practical, data-informed guidance to health workers and supervisors. This enables them to identify which children should be prioritised, determine when follow-up is most urgent, and decide where outreach efforts should be focused. The CDST does not replace health workers’ judgement—it strengthens it by supporting more structured, consistent, and informed decisions in real-world settings.

Core Functionalities

Identification and Reporting

VaxyReach empowers community health workers to systematically identify and report zero-dose and under-immunised children, ensuring that no child is ever missed at the local level.

Geospatial Mapping

The platform visualises the geographic distribution of missed children, supporting more precise and targeted microplanning at settlement and health area levels.

Targeted Microplanning

VaxyReach generates prioritised action lists for outreach activities and supports the effective allocation of health workers and resources to reach zero-dose and under-immunised children.

Follow-up and Monitoring

The system enables continuous tracking of vaccination status over time and allows for real-time updates of individual child records during and after interventions.

Operational Dashboards

VaxyReach provides real-time visibility for health facilities and district managers, supporting performance monitoring and enabling adaptive planning based on current data.

Community Engagement and Learning

The platform supports community awareness-raising on immunisation while providing structured learning content for vaccinators and community actors to strengthen knowledge and engagement.

VaxyReach has been developed and deployed in collaboration with national immunisation programmes and partners, with a focus on strengthening last-mile service delivery. 

It is designed to

  • Integrate with existing health information systems (including DHIS2 where applicable) 
  • Adapt to country-specific guidelines and operational realities 
  • Scale progressively from pilot to national level
